Criminal History Request Form:
(Requires Adobe Acrobat Reader) - use this form if you'd prefer to complete your form at home and
mail it with payment.
On-line Criminal History Request System:
- Use this site if you'd rather use the PA State Police automated system. The form and payment
are completed on-line. A credit card will be required.
